REPEAT VIOLATION A tuberculosis (TB) screening is required every two years from the last screening. Staff #4 had documentation of an updated TB screening one month after it was due and staff #5 had documentation of an updated TB screening four months after it was due.

The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child #3, who had been attending for over a month, did not have documentation of immunization records on file.

When food is brought from home the food container must be clearly dated. Several of the children's food containers in the 4-year-old and 5-year-old classroom were not labeled with a name or date.

A written care plan is required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Child #3, enrolled for over a month, has a diagnosed food allergy and did not have a written care plan in their file.

Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #3, enrolled for over a month, was missing the names, addresses, and phone numbers for two designated persons to contact in an emergency.

The center shall keep staff records with all the required information. Staff #1, who had been actively working at the center for the past three months, did not have documentation to demonstrate that the individual possesses the education, certification, and experience required by the job position.

Based on a review of staff records and interview, the vendor failed to ensure that each staff complete the Virginia Annual Health and Safety Update Training annually as required. Evidence: The record of staff #5 contained documentation of the annual update training dated 6/12/23. There was no documentation of the 2024 update training. Staff #7 confirmed the 2024 was not completed.

Based on a review of staff records and interview, the center failed to obtain an annual minimum of 16 hours of training appropriate to the age of children in care for each staff. Evidence: The record of staff #4 contained 12 hours of annual training from 3/25/23 to 3/24/24. Staff #6 confirmed 16 hours of annual training was not completed.

Based on a review of the fire inspection and interview, the center failed to obtain an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: The most recent documentation of the fire inspection is dated 8/28/23. Staff #6 confirmed the annual fire inspection was not scheduled.

Based on observation and interview, the center failed to ensure hazardous substances, such as cleaning materials, shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: There were two O'Cedar mops with the cleaning solution installed that were unlocked and accessible to children. The kitchen also contained cleaning materials that were unlocked. Staff #6 confirmed the cleaning materials were unlocked.
