Cribs, cots, rest mats, and beds when being used for sleeping or napping by children other than infants shall have linens consisting of a top cover and a bottom cover or a one-piece covering that is open on three edges. There were two children observed resting on their mats during naptime and their mats did not have bottom covers.

The trashcan that the provider used to dispose of a soiled diaper, was not foot-operated and did not have a lid.

There were 3 bottles of prescribed medications belonging to a household member unlocked on an end table by the couch accessible to children. 2.
