REPEAT VIOLATION When staff are supervising children, they shall always ensure their care, protection, and guidance. 1. The Preschool classroom was observed for approximately an hour, during which children were running around the classroom, throwing toys, climbing on the windowsill wedging themselves between the window and diaper table, standing on the arm of a small couch, standing on a bookcase, crying, and attempting to leave the classroom. Child #1 (age 3) pushed Child #2 (age 3) hard into the wall, making an audible thud when Child #2 hit his head against the wall. Child #2 began crying in front of Staff #1. Staff #1 did not respond to the incident and began to walk in another direction away from the children. A child put another child in a headlock. Staff #1 was sitting on the floor behind a bookcase and unable to see the children and Staff #2 was in the bathroom with children. Neither staff were able to intervene nor witness the incident. The child put in the headlock cried out loud while in the headlock. A child hit another in the face with a wooden block, causing that child to cry. Staff #1 was unable to witness the incident because the staff was sitting on the floor by a bookcase, which blocks sight of the children on the other side of it. Child #2 (age 3) chased child #3 (age 3) across the room and then pushed child #3 into the cubbies, causing child #3 to hit his head and cry. 2. Child #4 (age 2) had been receiving services at the center from an outside agency for several months on a weekly basis for approximately an hour each session. The center indicated that on occasion the individual from the outside agency is alone with child #3 for therapy. Administration did not have documentation of the individual?s name or agency or written parent authorization but allowed that individual into the center, the classroom and alone with child #3.

REPEAT VIOLATION Tables shall be sanitized before and after each use for feeding. In the Preschool classroom, some children were sitting at a table that had not been sanitized waiting for lunch. A child rolled a toy across the table, and a child wiped their nose with their hand and then touched the table, and lunch was then served on that table.

The licensing inspector tripped over a rug in the 2 year old classroom that was too large for the space and folded upwards against the counter, causing a tripping hazard.

In the Preschool classroom several children did not wash their hands before lunch, and one child was observed touching their nasal discharge and then proceeded to touch several surfaces before being served lunch.

In the Pre-k classroom there were numerous flying insects around the trash can and what appeared to be pieces of food on the floor around the trashcan. 2. The toilet in the girl's bathroom in the downstairs hallway had an odor, yellow liquid in several areas on the toilet seat, and used toilet paper under the rim. Staff #12 stated the toilet had overflowed the day before and the toilet was broken. 3. In the DP2 classroom there was visible dirt/crumbs on the diapering table.

The center?s procedures for identifying where attending children are at all times includes utilizing Name-to-Face when moving between locations. Before the staff cross the first and last threshold staff are to complete name-to-face checks. On May 16, 2025, Staff #1 and staff #2 did not complete name-to-face checks when moving from the playground to the classroom, leaving child #1 (two years old) alone outside for approximately 30 seconds.
