The following staff records shall be kept for each staff person: documentation that two or more references as to character and reputation as well as competency were checked before employment or volunteering; and information, to be kept at the center, about any health problems that may interfere with fulfilling the job responsibilities. Staff #1 did not have dated references in their file. Staff #1, Staff #2, Staff #3, Staff #4, and Staff #5 did not have a statement confirming health for job responsibilities.

Staff shall complete orientation training in subsection C of this section prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities. 1. Staff #1, who has been employed for more than 3 months, did not have documentation of completing orientation in their staff file. 2. Staff #5, who has been employed for more than 2 months, did not have documentation of completing orientation in their staff file.

The center shall provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. The center was unable to provide an up to date copy of a fire inspection dated within the last 12 months.

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. 1. Clorox wipes and Clorox bleach spray were unlocked but out of reach sitting on cabinets near cubbies and above the sink in the toddler room. 2. Bleach water solution was also found in the unlocked upper closet in the three's room out of reach of children.

When a child is placed in an infant seat or high chair, the protective belt shall be fastened securely. Staff #3 was observed feeding 4 infants lunch in the classroom. One infant was not buckled into the highchair during feeding.

Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the following information: Name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached. Child #1 and Child #2 were missing a second emergency contact. Child #3 was missing the address and phone number of their second emergency contact. Child #4 did not have any emergency contacts within the child's file.

The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. Child #5, under the age of two, did not have an updated copy of immunizations every six months. The last immunization record is dated 5/28/2024.

Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of child #5, who had been enrolled for over six months.

Each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with the children. Staff #1, employed for more than two months, did not have documentation of a completed tuberculosis test.
