On February 2, 2026, Child #1 (age seven months) received second degree burns on the side of the left leg (from mid-thigh to the top of the ankle) while in care of the center. Interviews revealed the burn is believed to be a result of the hot water from the bottle warmer dripping off the bottle onto the child?s leg as staff noticed the child's pants leg was wet and did not change the child, nor further investigate. The water temperature in the bottle warmer measured as high as 211 degrees Fahrenheit.

Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. Staff #1?s TB was dated seven weeks prior to date of employment.

The Virginia Department of Education ? sponsored orientation course shall be completed within 90 days of employment. Staff #5, who is working with children, had not yet complete this course which is over due by four days.

When staff are supervising children, they shall always ensure their care, protection and guidance. On April 22, 2025, Staff #1 sat Child #1, age 12 months, on a shelf and stepped seven feet away. Child #1 fell 3 feet to the bare floor resulting in the child crying.

Staff who work directly with child shall annually attend at least 16 hours of training to include the department?s health and safety update course. Staff #3 last completed the department?s health and safety update course on 03/02/2023.

Staff shall complete annually a minimum of 16 hours of training appropriate to the age of the children in care. Staff #3 completed 11.5 hours of annual training for the 2023-2024 training year.

Medication shall be labeled with the child?s name, the dosage amount, and the time or times to be given. There was one prescription medication that was not labelled with the required information.
