The foot operated trash can, was placed on an elevated surface, where the foot pedal was not available to the staff member in the toddler room.

Staff and children's hands shall be washed with soap and running water after diapering. A teacher in the two year old classroom was observed changing a child's diaper, and then allowing the child to go lay on their cot without washing their hands. The teacher was also observed tending to other duties without first washing their hands as well.

The changing tables in both the infants and toddler classrooms had locking mechanisms on their cabinet door to prevent access to cleaning materials, however the locking mechanisms were broken.

The center is required to obtain documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children, and shall have been completed within the last 30 days prior to employment. Staff #4 had a TB screening that was dated over 90 days prior to their first date of employment. Staff #5 had a TB test dated over one week after their first date of employment.

Evidence: Child #1 was injured while in the center's care on 9/4/2024. The center was notified of the result of the injury on 9/5/2024, however the department was not notified until 9/9/2024.
