Systemic Deficiency Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. On one wall in classroom #1, two areas were observed to have peeling paint. One of the areas with peeling paint had pieces of the dried paint on the ground below. On the school-age and preschool playgrounds the poured-in-place resilient surfacing had areas that were cracked and had pieces of the surface peeled off, posing as potential tripping hazards. On the Preschool playground, the concrete sidewalk and the resilient surfacing had separated, the resilient surfacing is sitting slightly higher than the concrete, posing as a potential tripping hazard to children entering onto the resilient surfacing. On the school age playground a plastic picnic table was cracked, exposing sharp plastic.

The training shall include an in-person competency demonstration. Staff #1, who had been employed for over two weeks, was working alone with a group of children (all age: 3 years), and did not have a current certification in CPR and first aid that included an in-person competency demonstration.
