When staff are supervising children, they are required to always ensure their care and protection. On 4/22/26 between approximately 5:45pm and 5:56pm, Child A, four-years-old, exited the playground via an emergency exit gate. Child A walked across the center?s parking lot, through a short, wooded path, crossed two additional parking lots, a two-lane road, and two more parking lots to a playground that was measured to be approximately 0.2 miles from the center. Staff #1 and Staff #2 were responsible for the children on the playground and did not notice that Child A was missing until a witness approached the building and informed management that they had seen a child exit the playground.

Areas and equipment of the center, inside and outside, are required to be maintained in a safe and operable condition. On 4/22/26, the alarm on the emergency exit gate of the playground did not go off when a four-year-old child pushed the bar and exited the playground alone. Staff reported that both the alarm bar and the top latch were not in working order for the child to be able to exit the gate and the alarm not sound.

The center is required to maintain a written record of children?s serious and minor injuries in which entries are made the day of occurrence. There were no injury reports documented for children in the Preschool 1 and PreK classrooms who were bitten within the past six weeks.

On 4/22/26, Child A, four-years-old, exited the center?s playground alone between 5:45pm and 5:56pm via an emergency exit gate. Child A was unsupervised until staff located the child approximately 0.2 miles from the center at 6:11pm.

On 12/17/25 preschool-aged children were observed playing on an indoor climber in the gym that measured 47 inches tall at the highest point. There was no resilient surfacing under or around the climber.

The center is required to obtain documentation of additional immunizations once every six months for children under the age of two years. 1. The most recent immunizations on record for Child B, 17-months-old, were dated when the child was eight weeks old. 2. The most recent immunizations on record for Child D, 22-months-old, were dated when the child was eight weeks old.

On 12/4/25 at approximately 10:35am, four children were observed in the Infant A classroom. The attendance record contained documentation of three children present. 2. On 12/4/25 at approximately 10:52am, four children were observed in the Toddler A classroom. The attendance record contained documentation of three children present.

The trash can in the bathroom of the Two A classroom did not have a lid and was observed to contain dirty diapers.

The center is required to ensure that at least one staff in each classroom or area where children are present have current certification in cardiopulmonary resuscitation (CPR). On 8/20/25, Staff #1 was observed to be working alone in a classroom with children. Staff #1 does not have current certification in CPR.

On 5/12/25, two medications were observed in an upper unlocked cabinet in the Preschool 1-B classroom for children two- to three-years-old. 2. Three other medications were observed in unlocked desk drawers in the center lobby.

(Systemic Violation) Medication is required to be labeled with the child?s name, the dosage amount, and the time or times to be given. On 5/12/25 a medication was observed on-site without a label indicating for which child the medication belonged. Staff indicated the medication was for Child F.

(Systemic Violation) Written authorization from the parent is required for prescription and nonprescription medication. One prescription medication was observed on-site for Child G and one nonprescription medication was observed on-site for Child F. There were no written authorizations from the parent for either medication.

The center is required to provide to the licensing inspector an annual fire inspection report. The annual fire inspection was more than 5 months past due.

The center is required to maintain a written record of daily attendance for each group of children that documents the arrival and departure of each child in care as it occurs. 1. At approximately 11:05am on 5/12/25, there were 14 children observed in the Preschool 2B classroom for children three- to four-years-old. The attendance record contained documentation of 13 children present. 2. At approximately 11:20am on 5/12/25, there were eight children observed in the 2-A classroom for children two-years-old. The attendance record contained documentation of six children present. A staff member updated the attendance to include the two missing children. The same staff then brought another child to the classroom and did not add the child to the attendance record. 3. At approximately 11:38am on 5/12/25, two children were observed in the Toddler-B classroom for children 12- to 18-months old. The attendance record contained documentation of three children present. 4. At approximately 11:20am on 5/12/25, the attendance record for the Preschool 1-B classroom for children two- to three-years-old contained documentation of children present. There were no children in the classroom. The children had been divided between several classrooms and were not transferred to the attendance in those classrooms.

The center is required to maintain a current written list of all children?s allergies, sensitivities, and dietary restrictions in each room or area where children are present. On 5/12/25 in the Preschool 2A classroom, the Toddler B classroom, and the Toddler A classroom, the posted allergy list was dated 2/3/25 and contained documentation that there were no allergies in the classroom. The current allergy list for the center was dated 5/4/25 and included dietary restrictions and sensitivities for children in the Toddler B and Toddler A classrooms.
