The center shall maintain a written record of children's serious and minor injuries in which entries are made on the day of occurrence. The record shall include the date and time when parents were notified. Eight injury reports were reviewed during the inspection. Seven of the eight injury reports did not include the time when the parents were notified.

Each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis (TB) screening. The documentation for the screening shall be submitted at the time of employment and prior to coming into contact with children. The record for Staff #4 did not contain documentation of a negative TB screening. Staff #4 has been employed for 10 days, supervising children.

When food is brought from home, the food container shall be sealed and clearly dated and labeled in a way that identifies the owner. Eight water cups located in Classroom #4 were not labeled or dated. The eight water cups were provided from the children?s homes.

Prescription and nonprescription medication shall be given to a child, only with written authorization from the parent. Medication was present at the center for Child #7. The medication was given to Child #7 daily and documented on the medication log. There was no written authorization on file signed by the parents.

The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. Classroom #1 had a floor mat that contained multiple tears in the equipment. Classroom #2 had an area of peeling paint underneath the window. The area was accessible to the children.

Hazardous substances must be in a locked place that prevents access by children. The unlocked closet located in Classroom #3 contained three cleaning chemicals labeled ?Keep Out of the Reach of Children?.
