The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. There was no physical exam records available for Child #3, who has been in care for 4 months.

A tuberculosis (TB) screening is required for adult household members every two years from the last screening. An updated TB screening for Household Member #1 was not completed and is overdue by 1 month.

The vendor shall maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs. The vendor's attendance record showed 9 children present at time of inspection, there were 12 children present. Vendor stated all children had arrived approximately 4 1/2 hours prior to inspection.

The record for Child #2 was missing written authorization for emergency medical care or a written objection to provision of medical treatment on religious or other grounds. The child was present during the inspection and had been in care for a month.

Caregivers shall obtain a minimum of 16 clock hours of training annually. Caregivers #2 and #3 did not have documentation of obtaining any training hours in 2024. Caregiver #3 stated they did not do any trainings in 2024.
