The record for Staff #4 (employed for over two years) was missing the address of an emergency contact. The record for Staff #5 (employed for over two months) was missing the address of an emergency contact.

Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts A video was reviewed for an incident that occurred on October 8,2025, showing staff member #1 approaching child #1 (age: 18months) after child #1 took another child's water bottle. Staff member #1 got down on the child's level, grabbed child #1 on both upper arms and in loud voice said, "No throwing." Child #1 was visibly upset.

Staff shall wash their hands with soap and running water before and after helping a child use the toilet and before and after a diaper change. Staff # 6, classroom #1, did not wash their hands before and after helping a child use the toilet. Staff #7, classroom #2, did not wash their hands before and after a diaper change.

Disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Staff # 6, classroom #1, used their hands to lift the lid of the storage system to disposed of an soiled diaper instead of using the foot-operated system.

Over-the counter skin products shall not be kept or used beyond the expiration date of the product. Three diaper creams were kept beyond the expiration date of the product. Classroom #1 had two diaper creams expired. Child # 6's diaper ointment expired on two months ago. Child #7's diaper ointment expired 6 months ago. Classroom #2 had one diaper cream expired. Child #8's diaper ointment expired three months ago.
