On the toddler playground the fall zone for the yellow slide chute did not have the required 6 feet of fall zone because there was a border/ fence approximately 4 feet 4 inches from the slide chute.

In Classroom E a container of bleach and water mixture was observed in an unlocked cabinet. Containers were labeled ?keep out of reach of children? and at least one other statement, ?hazardous?, ?harmful? and ?flammable?.

Repeat Violation Hand washing and toileting procedures were not followed by Staff #6. 1.Staff #6 was observed changing a diaper for child #6 and the child's hands and staff hands were not washed with soap and running water after the diaper change. 2.Staff #6 was observed serving snack to Child #6 and the staff hands were not washed with soap and water before serving snack. 3. Child #6's hands were not washed with soap and water or a disposable wipe before eating snack.

On the Two?s playground the resilient surfacing at the base of the blue slide chute measured between 4-5 inches and a minimum of 6 inches is required.

Repeat Violation Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. 1.On the preschool playground there was a picnic table with splintered wood, chipped and peeling paint. 2 On the preschool playground there were two sections of exposed sharp concrete sticking out of the ground near the chalk wall. 3. In Room F there was a book shelf and diaper changing table that each had areas of splintered wood, chipped and peeling paint. 4. In Room A there was a diaper changing pad and table that had areas of dirt. Wood chips were observed under the diaper pad and the dirt on the diaper pad and table came off when wiped with a paper towel.

This list shall be dated and kept confidential in each room or area where children are present. 1.The allergy list in room #2 was posted in a way that people could see it and was not kept confidential. 2.The allergy list in room #4 did not include physicians / parent requested dietary restriction (no milk, nuts, eggs and fish) for Child #1.

Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. 1.The record for Child #1 did not have the name of the second parent listed or the appropriate legal paperwork shall be on file when the custodial parent requests the center not to release the child to the other parent. 2. The record for Child #5 did not have a written care plan as required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Child #5, has a diagnosed food allergy.

Children under 10 years of age shall always be within actual sight and sound supervision of staff except that staff need only be able to hear a child who is using the restroom. Child #3, age 3 was observed without sound supervision in the centers hallway restroom as Child #3 was alone and no other staff were in the hallway and all of the centers classrooms doors were closed.
