Each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and documentation shall have been completed within the last 30 calendar days of the date of employment. Evidence: Staff #1 had a TB screening that had been completed four months prior to employment, and not within the last 30 calendar days as required.

Cribs, cots, mats and beds used by children other than infants during the designated rest period shall have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges. Evidence: During the inspection the Licensing Inspector observed two children laying down for nap that did not have a bottom cover.

Prescription and nonprescription medication shall be given to a child only with written authorization from parent. Evidence: On October 8, 2025, at 2:56 p.m. Staff #1 applied a non-prescription skin product on Child #1 after observing an irritation on the child?s arm. The Non-prescription authorization form the center had on file expired two months ago. The center did not have current written authorization from the parent to apply the skin product.

Evidence: The record for Staff #2, Staff #4, and Staff #5 did not contain documentation of a repeat tuberculosis screening every two years. The records for Staff #2 had a tuberculosis screening that expired for one year before obtaining a new screening, Staff #4 had a tuberculosis screening that expired for nine months before obtaining a new screening, and Staff #5 had a tuberculosis screening that expired 14 months ago.

In the Noah?s Ark room, ages 6 weeks to 16 months-old, a mini fridge with a box fan sitting on top of it was observed. Upon further inspection it was discovered that the mini fridge was missing one of the leveling feet causing it to be unstable and posing an entrapment hazard. 2. In the Gift of the Spirit room, ages 16 months to two-years-old, children were observed playing on a carpet that had several large wrinkles, posing a trip hazard.

Evidence: When the three-year class utilize the restroom in the multi-purpose room after outside time, staff remain at the restroom and send children to hang up their coats in the three-year class. The three-year classroom is located outside of the multi-purpose room across from the Directors desk in the lobby.
