The center shall inform the departments representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention. Child 1 (23 months old) was dropped on their head while under the supervision of the center on 2/25/2026. The parent took the child for medical attention on 3/4/2026. The center did not notify the department about the injury requiring outside medical attention.

Behavioral guidance shall be constructive in nature and age and stage appropriate. Staff 6 did not use appropriate behavior guidance to redirect Child 1 (age 23 months) who did not want to clean up. In response to the child?s behavior, Staff 6 carried Child 1 upside down from Classroom #1 to place Child 1 in time out in Classroom #2. There were no attempts to guide the child to assist with clean up.

The following actions or threats thereof are forbidden: roughly handling a child. On 2/25/2026, Staff 6 carried Child 1 (age 23 months) upside down out of Classroom #1 into Classroom #2. Staff 6 swung Child 1 (age 23 months) horizontally to put them in a table bucket seat. Staff was unable to hold the child, and the child fell on their head on the tile floor.

The center shall maintain a written record of children?s serious and minor injuries in which entries are made the date of occurrence. Child 1 (23 months old) was dropped on their head to a tile floor while under the supervision of the center on 2/25/2026. Staff 6 gave the child an ice pack for the head. There was not a written record of the injury.

For children ages 16 months through preschool age, during the designated rest period, when children are resting or in an inactive state, rest period ratios are permitted to double if staff required by rest period ratios is within sight and sound at all times in the same space as the resting or sleeping children. One Staff Member was observed sitting in the hallway between two classrooms during rest period. There were ten 2-year old children in one classroom and ten 3-year old children in the classroom directly across the hallway. Staff Member was not within sight and sound of either classroom and was not in the same space as the resting or sleeping children.

Prepared infant formula shall be refrigerated, dated and labeled with the child's name. In the infant room refrigerator, there were 3 infant bottles containing prepared infant formula, that were not dated nor labeled with the child's name.

Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. Staff 3 TB was dated six months prior to date of employment.

The center shall develop written procedures for prevention of shaken baby syndrome or abusive head trauma, including coping with crying babies, safe sleeping practices and sudden infant death syndrome awareness. The written procedure for prevention of shaken baby syndrome did not include safe sleeping practices.

Staff are to complete the VDOE sponsored online orientation course within 90 days of hire. Staff 2 completed the course 24 days late and Staff 4 completed the course over one month late.

The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. Chipped paint was observed in many areas on the rails of the crib assigned to Child 2.
