Areas and equipment of the center shall be maintained in a clean, safe and operable condition. In the Pre-K classroom, a wall was observed with a section of chipping paint. In addition, the play kitchen was observed to be dirty.

In the toddler class, various cleaning materials were found to be unlocked on a shelf in the bathroom. Additional cleaning supplies were found to be unlocked in the First Steps classroom on a shelf above a table, and in the Pre-K classroom on a shelf in the bathroom.

Based on observation, areas and equipment of the center were not maintained in a clean, safe, and operable condition. Evidence: In the First Steps classroom, there was a hole in the wall near an outlet, within reach of small children.

Based on observation, prepared infant formula was not labeled properly. Evidence: Two bottles in the infant room did not have the date to be served on them.

Based on record review, the center did not ensure that staff working directly with children had completed the department's health and safety update course annually. Evidence: Staff #3 did not have a certificate verifying that they had completed the annual health and safety update training for 2023.
