Any child for whom emergency medications have been prescribed shall always be in the care of a staff member or independent contractor who has completed the required training for that specific emergency medication. The center agreed to administer emergency medications for C1, C2, C6, and C7. There were no staff in the building when licensing inspector arrived for an inspection that had the MAT training required to administer medications in the event the children needed them.

The plastic climbing fort with the attached sliding board did not have 6 inches of resilient surfacing around the fall zones and at the exit of the sliding board. Bare ground was observed at the end of the slide.

A music sound cube located on an upper shelf in the 2?s classroom had a power cord assessable to the children and if pulled could fall on a child.

Children?s hands shall be washed with soap and running water after toileting, or any contact with blood, feces or urine. Licensing inspector observed a staff member in the Infant 1 classroom changed two children?s diapers and not wash the children?s hands. The staff did not wash her hands before or after these diaper changes.

There shall be a least 12 inches of space between occupied rest mats. Licensing inspector observed children resting on cots in the Toddler and 2?s Room that were not 12 inches apart.

A crib railing in the Infant 2 room was observed to have peeling paint and an infant was observed in the crib. 2. Multiple plastic play structures in the playground were cracked (kitchen set and caterpillar) exposing sharp edges. 3. Playground gate had loose and expose wire creating a scratch risk to children.

Hazardous substances shall be stored in an area inaccessible to children. Chemical cleaning products were observed and accessible under the sinks in the Toddler Room and 2-year-old room.

The diapering surface shall be cleaned with soap and at least room temperature water and sanitized after each use. Staff in the 2-year-old room cleaned the diaper changing surface after diapering a child and sprayed it with a bleach water solution to sanitized the surface, but did not allow it to dry for a period of at least two minutes.

Children shall not be allowed to drink while walking around. A child was observed during the inspection drinking from a sippy cup while walking around in the Infant I/Infant II room.
