The records for child #1 did not contain documentation of an annual review with the parent the emergency contact information to ensure the information is correct and the parent s signed acknowledgment of the review which is overdue by more than 3 years. The record for child #2 did not contain the parent s signed acknowledgement of the receipt of the information regarding provisions of the family day home's emergency preparedness and response plan. The record for child #4 did not contain the name, address and phone number for one of two required emergency contact person to be called in the event of an emergency if a parent cannot be reached and did not contain the names of persons other than the custodial parents who are authorized to pick up the child.

Child #3 who was present during the inspection did not have an immunization record which was overdue by 4 months. Child #5, who was not present but reportedly may attend sporadically did not have an immunization record which was overdue by more than 9 years.
