This list shall be dated and kept confidential in each room or area where children are present. The School Age classroom did not have a written allergy list, while children were present in the classroom.

Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. In the School Age classroom, the light switch adjacent to the stage was missing a cover, exposing the wall's interior and screws. The Preschool and Prekindergarten playground had chipped, peeling paint on top of the fence railings. On the Infant, Toddler, and Two's playground, there was a small screw sticking out in the back of the hose box on the wall.

Child #6 had emergency prescription medication on site but there was no pharmacy label.

When an authorization for medication expires, the facility shall notify parents that the medication must be picked up within 14 days of the authorization expiring or that a new authorization should be completed. Child #6 had emergency prescription medication on site and the parental authorization expired over five months ago. The authorization had not been renewed and the medication was not returned or disposed of as required.
