Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. The laundry room/ storage room was left open there was a bucket of clothes detergent on the floor and there was unlocked cabinet with hazardous substances. In the three to five year old room, under one of the two sinks there was cleaner that had keep out of reach and warning. In the main open space, there was a unlocked cabinet with disinfectant aerosol spray.

Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles. There was a basketball hoop in front of the slide causing the slide not to have a 6 feet fall zone. The slide resilient surface was unable to be determine due to the mulch into shas turned into soil.

There shall be a posted daily schedule that allows for flexibility as children?s needs require. The center did not have a posted daily schedule for 3 to 5 year old classroom posted.

There shall be at least 12 inches of space between occupied cots, beds, and rest mats. In the 2 year old classroom, there were cots not spaced 12 inches parts.

Repeat violation The center shall maintain a written record of children's serious and minor injuries in which entries are made on the day of occurrence. Injury record #1 was missing date and time parent was notified. Injury record #2 was missing time parent was notified. Injury report #3 was missing method of preventing injury and time parent was notified. Injury report #4 was missing time parent was notified.

The center shall ensure that during transportation of children: Virginia state statutes about safety belts and child restraints are followed as required by ?? 46.2-1095 through 46.2-1100 of the Code of Virginia; There were two children who needed a booster seat. The center confirmed that there was a child on the vehicle who did not have booster seat when being transported.

Repeat violation Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Outside on the playground there was a fence that was detached by the AC unit had toys that were cracked and broken . There were multiple bolts that are raised around the mulch border that could cause tripping hazard. There is a track like area and the track mats are raising / buckling causing a tripping hazard.

Outside on the playground there was a broken grey truck with no wheel, broken toy push lawn mower with a broken wheel , broken plastic clear storage bin with sharp edge and a bolt raised on the border of the playground causing tripping hazard or entanglement..
