Child #1 (two years old) and Child #2 (three years old) were not provided immediate assistance to prevent them from leaving the building, resulting in the children being without sight and sound supervision from staff.

When supervising children, they shall always ensure their care, protection and guidance. On March 24, 2026, Child #1 (two years old) and Child #2 (three years old) exited the building. Child #1 ran into a 4-lane road resulting in a driver alerting other drivers. Child #2 ran to the side of the road. The children ran back towards the center before a staff member opened the front door calling them back inside.

On March 24, 2026, there was not a program leader or program director qualified staff member in the 2?s classroom after 3:30 pm when an incident occurred at approximately 3:45 pm where two children exited the building unsupervised.
