The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include the following: Date and time of injury; Name of injured child; Type and circumstance of the injury; Staff present and treatment; Date and time when parents were notified; Any future action to prevent recurrence of the injury; staff and parent signatures or two staff signatures; and documentation on how parent was notified. 1. Injury report #1, dated 1/13/2026, was missing future actions to prevent injuries. 2. Injury report #2, dated 1/15/2026, was missing appropriate documentation for how the parent was notified and future actions to prevent injuries.

A menu listing foods to be served for meals and snacks during the current one-week period shall be dated. The center's posted menu was dated 1/12/2026-1/16/2026.

The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. Child #1, who has been attending for over a year, did not have up to date immunizations in their file during the inspection on 1/22/2026.

The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. The center received documentation of Child #1's immunizations four days after beginning attendance.

The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. 1. The purple and yellow playground had multiple steps with exposed rusting metal where the rubber coating had peeled away. 2. The infant swings had spots of peeling paint that were not within reach of children.

A tuberculosis (TB) screening is required every two years from the last screening. Staff #5's last TB screening was dated 9/6/2022. The updated screening was due approximately 5 months ago. The updated screening has not yet been completed.
