At least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department, staff members and individuals from independent contractors shall obtain and submit the results of a follow-up tuberculosis screening. Staff #3 did not have an updated TB screening.

Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the required information. The record for child #2 did not contain the two required contacts. The record for child #3 did not contain one of the two required contacts.

A written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Child #1 and child #3 were both diagnosed with food allergies and there were no written care plans for either child.

Cribs, cots, mats and beds used by children other than infants during the designated rest period or during evening and overnight care shall have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges. Cribs when being used by infants shall have a bottom cover. 3 of 5 classrooms had some children napping without the required linens on their cots.
