Children three years of age or younger may not be offered foods that are considered to be potential choking hazards. The children in Classrooms 2, 3 and 4, ages 15 months-4 years, were served mini corndog nuggets that were not cut/sliced in a way to prevent the hotdog inside of the breading from being a potential choking hazard.

All over-the-counter skin products shall not be kept or used beyond the expiration date of the product. There were 5 over-the-counter skin products on-site that were expired.

For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. There were 7 children observed in Classroom 4. The written attendance for Classroom 4 documented 6 children in care.

The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. The immunization records for Child 3 (2 years), who has been attending for 13 months, were not updated once every six months.
