REPEAT VIOLATION For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. There were four children present in the infant B classroom, but five children were marked present on the attendance record. The Pre-School 1 classroom was combined with the Two's classroom for rest time for a total of 16 children in the classroom. There were 12 children marked present on the two's attendance record and eight children marked present on the Pre-School 1 attendance reflecting a total of twenty children.

The plastic golf ball cups on the put-put course of the toddler playground and school age playground are cracked with sharp edges exposed. There are two raised tiles on the splash pad posing a tripping hazard. The handwashing sink in the Two's classroom was clogged with approximately two inches of water in the sink. The toilets in the Two's bathroom had rusted screws exposed at the base of both toilets. The metal ring behind the toilets in the Two's bathroom are rusted.

When staff are supervising children, they shall always ensure their, care, protection, and guidance. On 9/04/2025, child A (Age 2 years) was receiving a diaper change by Staff A on a mobile changing table in the bathroom of the two year old classroom. Child A was in the seated position on the changing table and turned to retrieve a diaper wipe. While turning, the child pushed their hands on the wall which caused the changing table to move. The wheels on the changing table were not in the locked position. Child A subsequently fell between the changing table and the wall, hitting their forehead on the floor. Child A sustained a bump to the right side of their forehead. Child A was later taken to the emergency room for evaluation.
