A child was observed tripping, falling, and crying during the inspection when they ran from the mulched area and caught their shoe on the side of the concrete path.

The composite play structure on the large playground required six feet of resilient surfacing on all sides, that is at least six inches deep for wood mulch. At the bottom of each slide on the large playground the wood mulch was breaking down and measured less than two inches deep.

An updated form was overdue for Child E by more than nine months. An updated form was overdue for Child F by more than six months.
