A container of disinfectant wipes with warning a label, ?Keep out of reach of children,? was unlocked in the 2-year-old ?purple? classroom, within reach of children.

The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. There was a metal pole and sign with sharp edges in an area used for children playing outside. Child #1 was seriously injured as a result of tripping and falling into the metal pole. The injury required immediate outside medical attention.

The child?s record did not contain a medication authorization for one over-the-counter medication stored on-site for child #6. Child #6?s record contained an authorization for one prescription medication without the parent?s signature.

The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child #2 had documentation of immunizations received 5 days after their first day of attendance.

8VAC20-780-160-A

The center is required to obtain documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children. Staff #4 has a TB screening that was dated 37 days after their first day of employment.
