On 08/05/2025, staff served food provided by the center that contained an ingredient to which Child A (21 months old) was allergic. Child A experienced an allergic reaction, with symptoms including a red and puffy face. A picture of Child A and a list of their food allergies were posted in all classrooms and the kitchen. Collateral Witness #1 had completed the center?s severe allergy packet and provided medication for possible allergic reactions. Management was unaware Child A?s had medication located at the center. It took several minutes to locate the allergy medication. The center did not contact 911. Staff #2 administered the medication without current Medication Administration Training certification. The Child Allergy Procedure stated, ?All staff members working in the kitchen must ensure they are reading the ingredient labels to minimize the risk of allergen exposure.? Staff #1 reported telling the previous supervisor twice about the loose, handing fire extinguisher. The supervisor said a work order would be submitted, but the fire extinguisher was not repaired. Complaint: Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Staff #1 observed that the fire extinguisher attached to the wall near the child-size sofa in Classroom #1 was ?very loose and hanging?. Staff #1 heard Child A (two years old) cry out. The fire extinguisher had fallen off the wall and struck Child A on the side of the face and mouth. Child A had a red mark on the cheek and was bleeding from the mouth. The diapering area shall have a leakproof covered receptacle for soiled linens. After Child #5?s clothes became soiled, staff changed the child into clean clothes. However, the soiled clothes were left on the bathroom floor for about 20 minutes. During this time, other children entered and exited the bathroom while the soiled clothes remained on the floor.

Each center shall maintain and keep at the center a separate record for each child enrolled containing the required information. The record for Child #3 did not contain annual parent confirmation that the required information in the child's record is up to date. The last documented update to the child?s record was dated January 2023. Child #3 has been attending the center for two years and nine months. The records for Child #4 and Child #5 did not contain the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached. Child #4 has been attending the center for one month. Child #5 has been attending the center for one week.

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. Four containers of disinfectant and cleaning products were located in an unlocked cabinet underneath the diaper changing table in the Infant Classroom. The containers had warning labels, ?Keep out of reach of children,? . Infants were observed playing on the floor near the changing table.

REPEAT VIOLATION Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. The record for Staff #1 contained documentation of a TB screening obtained three days after employment. The record for Staff #2 contained documentation of TB screening obtained two months prior to beginning employment.
