The provider was out of compliance with R381-100-10(3)(a) by exceeding the mixed-age ratio of older toddlers and older children. In the classroom, there were 2 infants and 1 toddler mixed with 8 children three and older. Inspection: Unannounced, Annual Inspection. Correction status: Correction Verified. Correction action: On December 12, 2025, the licensor verified the director had separated the infants and toddlers from the older children in the classroom and ratios were maintained as stated in rule.

The Licensee was out of compliance with R381-100-11(9)(b) for failing to maintain security and supervision of every child and ensure each child was signed in and out. During an investigation, it was determined that a child was not signed out of the facility's care when they were picked up by a family member. This was a repeat non-compliance from 04/19/2023. Inspection: Complaint, Investigation Inspection. Correction status: Manager approval, no follow up needed. Correction action: On May 27, 2025, the licensor verified that the Licensee re-trained all staff on the sign in and out procedures, as required in rule.
