R430-90-19(7)(a)-(c) — Trampoline cushioning requirement

The provider was out of compliance with this rule by not ensuring that each accessible trampoline with or without a safety net enclosure was placed on appropriate cushioning. R430-90-13(4) — choking hazards

The provider was out of compliance with this rule by having choking hazards accessible to children younger than 3 years of age. In the downstairs accessible closet, there was a small hard piece of rubber on the ground. This was a repeat rule noncompliance Inspection: Unannounced, Annual Inspection. R430-90-7(4)(a)-(g) — Caregiver requirements

The provider was out of compliance with this rule by having caregivers caring for children without having taken the CCL pre-service training. At the time of the inspection, two caregivers were caring for children without having taken the CCL pre-service training. This was a repeat rule noncompliance.
