The Licensee was out of compliance with R381-100-11(1)(b) by failing to ensure that each caregiver provided and maintained active supervision of each child. During an inspection, the licensor identified that 2 children were left unsupervised in the center's van after returning to the center from a school pick up trip. Inspection: Complaint, Investigation Inspection. Civil money penalty: $1000. Correction status: Manager approval, no follow up needed. Correction action: On June 3, 2025, the licensor verified that the Licensee had revised transportation policies and re-trained staff on transportation procedures, as required in rule.

Inspection: Unannounced, Annual Inspection. Correction status: Correction Verified. Correction action: On May 07, 2025, the licensor verified the director had moved the table out of the use zone, and the use zone was now 6 feet around the climbing dome as stated in rule.
