The Licensee was out of compliance with R381-100-11(1) by failing to ensure all children received active supervision. During an investigation, it was determined that two children engaged in inappropriate, sexual play, while two caregivers attended to other facility tasks. Inspection: Complaint, Investigation Inspection, Focus Inspection. Correction status: Correction Verified. Correction action: On June 26, 2025, the licensor verified that the provider re-trained all staff to ensure they provided and maintained active supervision to all children, as required in rule.
