R430-90-13(11)(a)-(b) — Live electric wires, electric outlets, surge protectors

The Licensee was out of compliance with rule R430-13(11) by having electrical outlets without protective covers when not in use. In the living room and upstairs bedroom, there were outlets without protective covers. This was a repeat rule noncompliance as noted on 3/4/2024. Inspection: Unannounced, Annual Inspection. The Licensee was out of compliance with R430-90-15(1)(a)-(f), by having unsanitary areas and a build up of food in the facility. In the kitchen, dining area, and play area, there was a build up of old food debris and spills on the floors, tables and countertops. Inspection: Unannounced, Annual Inspection. The provider was out of compliance with this rule by not ensuring that there were at least two caregivers when there were more than three children younger than two years of age.
