The Licensee was out of compliance with rule R430-90-8(2)(a)-(c) by not submitting an online background check form and fingerprints for an individual involved in the child care. At the time of the inspection, there was a caregiver who did not have a background check and fingerprints submitted Inspection: Unannounced, Annual Inspection. Correction status: Correction Verified. Correction action: On June 17, 2025, the licensor verified that the Licensee ensured a new background check form and fingerprints were submitted for all covered individuals as stated in rule.
