The Licensee was out of compliance with rule R430-90-19(13) by having strangulation hazards on or within the use zone of stationary play equipment. On the stationary play equipment there were 4 open c-hooks on the swing set. This was a repeat rule noncompliance as noted on 10/24/2022. Inspection: Unannounced, Annual Inspection. Correction action: On April 23, 2025, the licensor verified that the Licensee ensured there were no strangulation hazards on or within the use zone of stationary play equipment as stated in rule.

R430-90-8(3)(a)-(c) — New background check form and fingerprints for updates

The Licensee was out of compliance with rule R430-90-8(2)(a) by having a covered individual without fingerprints. At the time of the inspection, one covered individual had turned 18 years old and did not submit a new background check form with fingerprints. Inspection: Unannounced, Annual Inspection. Correction status: Correction Verified. Correction action: On May 5, 2025, the licensor verified that the Licensee ensured a new background check and fingerprints were submitted for all covered individuals as stated in rule.

R430-90-20(2)(a)-(e) — Vehicle used for transporting children

The provider was out of compliance with this rule by not having required supplies in the vehicle. At the time of the inspection, the provider was missing tweezers & bandages in the first aid kit. This was a repeat rule noncompliance. Inspection: Announced, Annual Inspection. Correction status: Correction Verified. Correction action: On 11/20/2024, the licensor verified that the provider ensured the vehicle used for transportation has a first aid kit with tweezers, band aids & antiseptic as stated in rule.
