The provider was out of compliance with Rule R381-100-24(13)(b) by not ensuring that breast milk brought from home for an individual child's use was labeled with the date and time of preparation or arrival time on it. In the Butterflies room, the breast milk bottles in the refrigerator were not labeled with the date they were prepared and the time they were prepared or had arrived at the facility. This was a repeat rule noncompliance as noted on 11/17/2025 and 5/21/2026. Inspection: Follow-Up Inspection. Repeat noncompliance. Correction status: Correction Verified. Correction action: On 6/12/2026, the licensor verified that the infant was not in care and had no bottles onsite, so this is corrected as stated in rule.

In the refrigerator of the Butterflies room, there were three bottles of breast milk for one infant that had not been labeled with the date and time of preparation time or the arrival time on them. This was a repeat rule noncompliance as noted on 11/17/2025. Inspection: Announced, Annual Inspection. Correction status: Correction Verified. Correction action: On 5/21/2026, the licensor verified that the caregiver labeled the three bottles with the current date and the time they had arrived at the facility that morning as stated in rule.

R380-80-5(10) — Services and supervision to meet each clients needs

The provider was out of compliance with R380-80-5(10) by failing to provide adequate supervision based on the child's abilities. During an inspection on 1/29/2026, the investigator identified that a school-age child was transported to the incorrect elementary school. This resulted in the child walking away from the school premises without supervision. Inspection: Complaint, Investigation Inspection. Civil money penalty: $200. Correction status: Manager approval, no follow up needed. Correction action: The licensor verified that the child was found and returned to their parent. The center has implemented a check off list for drop off and pick ups, to ensure is transported to the correct school.

R381-100-20(2)(a)-(e) — Vehicle requirements

The provider was out of compliance with this rule by not ensuring that each vehicle used for transporting children contained first aid supplies, including at least antiseptic, bandages, and tweezers. In one of the vans used for transporting the children, there were no first aid supplies. This was a repeat rule noncompliance. Inspection: Unannounced, Annual Inspection. Correction status: Correction Verified. Under appeal. Correction action: On 10/17/2024, the licensor verified that the director placed the required first aid supplies into the van as stated in rule.
