During the inspection, the investigator was able to confirm that a 4 year old child left the facility without the provider’s awareness on October 15, 2025. Inspection: Complaint, Investigation Inspection. Civil money penalty: $500. Correction status: Manager approval, no follow up needed. Correction action: On November 04, 2025, the investigator completed an inspection of all classrooms and verified appropriate supervision and ratios as required by rule.
