The Licensee was out of compliance with rule R430-90-8(2)(a)-(c) by not completing background check requirements. At the time of the inspection, there were two new household members without completed background checks. Inspection: Announced, Annual Inspection. Correction status: Correction Verified. Correction action: On July 21, 2025, the licensor verified that the Licensee ensured covered individuals had completed the background check process as stated in rule.
