In the outdoor area, there were s-hooks on the bottom of the swing with openings pointing out and up that were bigger than .04 inches in diameter. Correction action: On November 26, 2025, the licensor verified that the Licensee ensured the S hook on the swing was fully covered ensuring there were no strangulation hazards on the play equipment as stated in rule.
