R430-90-19(13) — Strangulation hazards

The Licensee was out of compliance with R430-90-19(13) by having strangulation hazards on stationary play equipment. In the outdoor area, there were s-hooks on the bottom of the swing with openings pointing out and up that were bigger than .04 inches in diameter. This was a repeat rule noncompliance as noted on 11/05/2024. Inspection: Announced, Annual Inspection. Repeat noncompliance. Civil money penalty: $400. Correction status: Correction Verified. Correction action: On November 26, 2025, the licensor verified that the Licensee ensured the S hook on the swing was fully covered ensuring there were no strangulation hazards on the play equipment as stated in rule.

At the time of the inspection, two children's admission and health assessment forms could not be located. This was a repeat rule noncompliance as noted on 3/14/2023.
