The Licensee was out of compliance with R381-100-11(1)(a) by failing to be physically present in a room or area with a child . During an inspection it was determined that a caregiver closed the door behind a child, who was left in the playground area. Inspection: Complaint, Investigation Inspection. Correction status: Correction Verified. Correction action: On June 24, 2025, the licensor verified that the provider re-trained all staff on active supervision of school age children, as required in rule.
