   The provider was out of compliance with Rule R381-100-13(9)(c) by not ensuring that hazardous chemicals, including any cleaner, and flammable, corrosive, and reactive material was stored in a container that was labeled with the contents of the container. In the bathroom of the Green Bears room, there was a bottle of hydrogen peroxide and water that had not been labeled with the contents of the container. This was a repeat rule noncompliance as noted on 8/7/2023. Inspection: Unannounced, Annual Inspection. Correction status: Correction Verified. Correction action: On 7/10/2025, the licensor verified that the director labeled the container with the contents of the container as stated in rule.

   The provider was out of compliance with Rule R381-100-13(8) by not ensuring that standing water that measured two inches or deeper and five by five inches or greater in diameter was inaccessible to children. In the infant/toddler room, there was a bucket of standing water on the side of the sink, accessible to children. Inspection: Unannounced, Annual Inspection. Correction status: Correction Verified. Correction action: On 7/10/2025, the licensor verified that the director made the standing water inaccessible as stated in rule.
