In the toddler/2's room, there was a mixed age group of children which included younger toddlers, older toddlers and two year olds that had more than eight children in the group. Correction action: On 10/8/2025, the licensor verified that the director designee had the children in the previous group separated into different groups so that they were in ratio and did not exceed group size as stated in rule.

R381-100-11(1)(a)-(f) — Provide and maintain active supervision

The provider was out of compliance with this rule by not ensuring that caregivers provided and maintained active supervision of each child, including the caregivers knowing the number of children in their care at any time. In the Toddler room, the caregiver did not know the number of children in their care. Inspection: Unannounced, Annual Inspection. Correction status: Correction Verified. Correction action: On 2/10/2025, the licensor verified that the caregiver recounted and then knew the number of children in their care as stated in rule.

The provider was out of compliance with this rule by not ensuring that for mixed-age groups of children, that they separated any single-age group that reached their maximum group size from the mix. In the Toddler room, the mixed age group of young toddlers, older toddlers and 2 year old children exceeded the maximum group size for the young toddlers. Inspection: Unannounced, Annual Inspection.
