In the facility, a caregiver tossed a child in the air, causing the child to fall to the ground and sustain a serious injury to their head. Inspection: Focus Inspection. Civil money penalty: $1500. Correction status: Correction Verified. Correction action: All staff members were re-trained and trained specifically to this rule. Scenarios and discussion and learning were part of this training. Staff signed for their training. The provider had documents to show training was completed. Staff also confirmed they were trained when spoken with on site. Staff directly involved completed additional hours of documented training.
