The Licensee was out of compliance with R381-100-11(1)(a)-(f) by failing to ensure all caregivers provide and maintain active supervision of each child. During an investigation, the licensor observed a caregiver step out of the infant/toddler classroom, and pull the door closed behind them, to do a diaper change in the hallway. Inspection: Complaint, Investigation Inspection. Correction status: Manager approval, no follow up needed. Correction action: On March 18, 2025, the licensor verified the director designee would ensure that caregivers are re-trained to ensure for children younger than five years old have a caregiver that is physically present in the room or area with the children, as required by rule.
