The Licensee was out of compliance with R430-90-6(9) by not ensuring that a parent completes an admission and health assessment form for their child before the child is admitted into the child care program.. At the time of the inspection, two children enrolled did not have completed admission and health assessment forms. This was a repeat noncompliance as noted on 10/23/2024 and 08/14/2025.
