It was observed that a 13-month-old was using playground equipment that is only approved for children ages 2 and up. — Visit: QRIS Monitoring visit,Technical Assistance,Complaint Investigation — Action taken: The incident was addressed immediately. Staff were promptly redirected and re-informed of the requirement that children must only use playground equipment that is age and developmentally appropriate. Leadership reviewed active supervision expectations with all staff, emphasizing proper positioning, awareness, and monitoring to ensure children remain in designated areas and are using appropriate materials at all times. Additionally, classroom and playground procedures were reviewed to ensure alignment with DHS regulations, and immediate adjustments were made to reinforce appropriate supervision and equipment usage across the program.

Staff member failed to report an abuse/neglect allegation to the Department of Children Services. — Visit: Complaint Investigation,Technical Assistance — Action taken: On site staff was educated on mandated reporting and facility contacted CCR&R for additional training. They will be conducting refreshers at quarterly staff meetings regarding mandated reporting; leadership to better discuss our role as mandated reporters during orientation process; documentation of any parent conversation and follow-up; additional trainings with CCR&R during our Teacher Work Week .
