1240-04-01-.09 Incident Reporting

During an unannounced visit to investigate a complaint, after interviews with educator and director it was determined that educators didn't inform parent of child being bit until the next day after the occurance. — Visit: Safety Plan Monitoring,Incident or Injury Investigation,Complaint Investigation — Action taken: The employee received a written reprimand. McNeilly had a conversation with parent. We conducted a follow -up conversation with parent. McNeilly staff will receive a daily page reminder about incident reports and parent notifications. The McNeilly staff involved with incident will receive Pro Solutions training on incident reporting and all new staff. Certificates will be emailed to you upon completion. All staff will review the four steps to reporting incidents. Parent notification, Brightwheel message, complete incident form and notify a program director.

1240-04-01-.11 Supervision

During an unannounced visit to investiage an incident, after interviewing educator and watching video footage LC determined that a child was left unattended in the hallway for 51 seconds. — Visit: Incident or Injury Investigation — Action taken: During busy transition time, a member of the management team will be available to assist classrooms as needed. Teachers in rooms 8,9,10, and 11 will utilize the walkie-talkie to notify staff that they are exiting the room and when they return to the room during transition periods. This will alert a member of the management team to go provide support in the preschool building. The teacher and all new staff will be attending the name to face training offered by DHS. A conversation was had with the teacher about the importance of using the name to face form during every transition. The teacher has received a written reprimand and was placed on a 90 probation period. The teacher and all new staff will attending the name to face training offered by DHS.

A lapse of Supervision of a 3 year old was not reported to the DHS till two days after the occurance. — Visit: Incident or Injury Investigation — Action taken: Executive Director met with Program Directors and we devised a policy on reporting critical violations as needed. The person who is made aware of the violation will report the incident on the DHS portal and DHS hotline. The person with first-hand knowledge will report any critical issues as needed on the DHs portal and the Hotline. The person will let the team know that the incident was reported.

It is the responsibility of the agency management to ensure that all staff and educators are properly trained to report serious incidents according to license rules. — Visit: Incident or Injury Investigation — Action taken: We will continue to collect proper documentation and notify parents as required. We will continue to have open lines of communication among the team. The person with first knowledge of the critical incident must report the violation via the DHS portal and hotline as needed. The agency will have a debriefing meeting afterward to ensure that violations have been properly reported. During an unannounced visit to investigate a complaint, after interviews with educators it was determined that a 4 year old male child's parent was not told of his eye being puffy and swollen before he was picked up for the day. — Visit: Complaint Investigation — Action taken: The educators will ensure that a proper health check is performed on children throughout the day, especially before children are transitioned in the afternoon. Educators will take a ProSolutions training and will meet with the Program Directors to put additional processes in place. This meeting will occur today 11/1/24. Educators will communicate with each other during the transitional periods to see if any updates or parent contacts need to be made prior to the transition occurring. Educators will take additional allergy/allergic reaction training on ProSolutions. Program Directors will meet with staff to keep them abreast of proper parent contact. They will also notify Program Directors as needed.
