    During an unannounced follow up visit, LC Walker observed breakfast in several classrooms with blueberries being served and not cut, diced or mashed. Also, in the Pre K one (3.5-4 year olds) classroom the educator in the classroom was serving the children breakfast and was not seated while the children had their food in front of them (no child had started eating yet). — Visit: Follow-up visit,Safety Plan Monitoring,Technical Assistance — Action taken: The cook will ensure that all blueberries will be pureed prior to serving to the Infant Toddler classrooms- and mashed for the Preschool PreK Classrooms. All Staff will call for help if they are in a one teacher ratio classroom during mealtime to ensure supervision is properly followed.

    During an unannounced follow up visit, LC Walker observed a 1:20 Adult:Child ratio in the 4-5 year old classroom when the ratio should have been 1:16. — Visit: Follow-up visit,Safety Plan Monitoring,Technical Assistance — Action taken: Agency will have extra staff available at high traffic drop off/ pick up times. Agency's admin will check classroom ratios with 15 min headcounts during the peak hours as well to ensure compliance for Adult:Child Ratios. Center staff will take a training on supervision conducted by CCR&R with upper Admin conducting another training with all staff on Supervision as well.

    During an unannounced visit on 01/29/25 while serving a safety plan, LC Walker observed two (2) children in the Toddler Three class at the table eating breakfast while the rest of the class was with an educator about seven (7) feet away at free play. The second educator in the classroom walked away from the children at the table into a closet about two (2) feet away. — Visit: Safety Plan Served,Technical Assistance — Action taken: All educators will receive mealtime supervision training from managment and targeted training from CCR&R. The Agency will have all educators read, sign and date the Mealtime Supervision section page in the Rules and Regulataions to keep in staff files.

    Based on the director and staff's interviews, the incident of a three-year-old male child with both eyes swollen during naptime in the Preschool Two class occurred on 01/15/25. The agency informed the parents of the incident around 1:15 pm. The written incident report was not provided to the authorized representative of the child at the time of pick up. The child no longer attends the agency. — Visit: Complaint Investigation,Technical Assistance — Action taken: The agency condcuted training with staff on Incident Reporting with CCR&R coming out to do targeted training for management and staff.
