During a complaint investigation, while LC Kelley was obtaining ratios for the classrooms, a medication in the PreK One class had expired in March 2026. — Visit: Complaint Investigation — Action taken: To prevent this violation in the future, the provider will conduct monthly medication audits. Staff also refreshed on medication handling and expiration monitoring.

During a complaint investigation, LC Kelley was made aware that incident reports had not been filed out. — Visit: Complaint Investigation — Action taken: Staff participated in a refresher training on incident reporting procedures.

1240-04-01-.12 Health and Safety

During a complaint investigation it was confrimed through video that a verbal and physical altercation took place in the building. Children were not involved but could hear the altercation, there were parents that witnessed the incident — Visit: Technical Assistance,Complaint Investigation,Incident or Injury Investigation — Action taken: Provider will continue to review the code of conduct with staff. Provider will follow policy and procedure for termination per the employee handbook

1240-04-01-.15 Program Language and Literacy Development

During complaint investigation it was discovered that an educator was using foul language toward the children and expressing her feeling about the children in front of them. — Visit: Technical Assistance,Complaint Investigation,Incident or Injury Investigation — Action taken: CCR&R will be conducting training on stressful situations and emaotional impact. Provider will continue to adhere to the policy and procedures in the employee handbook

1240-04-01-.11 Supervision

During the self-reporting incident investigation, video footage was observed that showed the educator of the infant two room stepping across the threshold leaving the infant unsupervised. There was another educator that came into the room shortly after who was also standing at the threshold However there was not a direct supervision person inside the classroom. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: Provider immediately retrained on active supervision policy and procedures as well as maintaining sight and sound of all children at all times. Provideralso implemented a sign in/out procedure for all classrooms that includes ratios and number of children present that staff is required to sign at every transition. Administration is also doing active safety checks throughout the building during number counts every hour on the hour.
