    During the self-reporting incident investigation, video footage was observed that showed the educator of the infant two room stepping across the threshold leaving the infant unsupervised. There was another educator that came into the room shortly after who was also standing at the threshold However there was not a direct supervision person inside the classroom. — Visit: Incident or Injury Investigation,Technical Assistance — Action taken: Provider immediately retrained on active supervision policy and procedures as well as maintaining sight and sound of all children at all times. Provideralso implemented a sign in/out procedure for all classrooms that includes ratios and number of children present that staff is required to sign at every transition. Administration is also doing active safety checks throughout the building during number counts every hour on the hour.
