During an unannounced visit to investigate a lack of supervision incident that occurred on 6/26/26 after speaking to educator it was determined the a 3 year old was left outside on the roof play area of the agency. The door closed and the child had not made it inside. The child was outside approximately 3-4 minutes. — Visit: Incident or Injury Investigation — Action taken: N/A

1240-04-01-.11 Supervision

During an unannounced visit to investigate a lack of supervision incident that occurred on 6/26/26 after speaking to educator it was determined that a 3 year old was left outside on the roof play area of the agency. The door closed and the child had not come inside. The child was outside approximately 3-4 minutes. The door leading to the roof locks from the inside once closed. — Visit: Incident or Injury Investigation — Action taken: N/A

1240-04-01-.15 Program Language and Literacy Development

During a visit to investigate a complaint after speaking with educators, it was determined that an educator was seen forcing a child to lay down to take a nap on two different days. — Visit: Complaint Investigation — Action taken: We will provide extra training for staff on how to care for infants and safe sleep for infants. I have spoken with the employee about how to gently place the children on the floor and crib. We talked about how to pick children up correctly. We also talked about how to lay children down for nap and how you can not force a child to take a nap. I explained that if a child does not fall asleep after 15 minutes you have to take them out of the crib. I will be placing a McKendree employee in with her in the classroom to have consistency in the room.

1240-04-01-.10 Duty to Report Child Abuse and Neglect

During a visit to investigate a complaint after speaking to assistant director it was determined that a reasonable suspicion of child abuse was not reported to DHS or DCS. — Visit: Complaint Investigation — Action taken: They will complete the training course to go over mandated reporter responsibilities and committing to compliance going forward. Assist in leading a staff refresher session on mandated reporting.

1240-04-01-.09 Incident Reporting

During a visit to investigate a complaint after speaking with the assistant director and educator it was determined that an incident report was not completed the day the incident occured and reported. — Visit: Complaint Investigation — Action taken: The educator was reminded that any injury or incident must be immediately reported to administration and documented using the center's official incident report form. We also talked about how we should document anything that happens to a child. All staff will receive refresher training on incident and injury reporting procedures, including the requirement that reports must be completed and documented on the same day the incident occurs.

1240-04-01-.06 Staff

When LC reviewed educator trainings it was found that the required annual training hours and block grant trainings were not completed by 9/30/25. — Visit: QRIS Monitoring visit — Action taken: The director will review all staff training reports. Then follow up with all staff to discuss what trainings and how many hours need to be complete in the licensing year. The agency will review staff training reports throughout the year to make sure staff have complete trainings. We will provide In-Service days throughout the year for staff to attend in person trainings and have the opportunity to complete trainings on prosolutions.

1240-04-01-.11 Supervision

The director reported that a 2.5 yr old, male child was able to get on the elevator unnoticed by the educators and went down to the first floor. — Visit: Complaint Investigation,Technical Assistance — Action taken: The child's family was notified. The incident was reported to our LC, the DHS Provider Portal, and the Abuse/Neglect Hotline. Daycare parents have been notified of the violation and signatures are currently being collected. Additional training and reminders for all staff regarding safety and supervision are being provided. The director and assistant director have been shadowing class transitions for each age to ensure children are being accounted for through name to face, frequent headcount, etc...We are adding this topic to our upcoming in-service day on Friday, September 29th and will review best practices regarding supervision, with a focus on elevator safety. Walkie Talkies have been given to the staff and will be used moving forward. Teachers were placed under a mandatory safety plan. Since notified, we have placed both educators with additional supervisory staff for the entirety of their shifts until the plan is lifted. We will continue to implement the strategies of frequent name to face/head counts during transitions and play. We will practice greater awareness and supervision of all children, particularly regarding entrances/exits/elevators during transitions/play. We will continue providing oversight and correcting any observed mistakes during transitions/play. We will now use walkie talkies for quicker, more efficient communication. We will continue to seek additional training opportunities to ensure related to supervision and better engagement with the children.

1240-04-01-.16 Physical Facilities

During an unannounced visit to investigate an incident after speaking to director LC determined that the upstairs 2nd floor walkway/hallway where children have been playing is not a DHS licensed space. — Visit: Complaint Investigation,Technical Assistance — Action taken: Classes were immediately informed that we would no longer be using the 2nd floor atrium space (other than the multipurpose/music room). No classes have used the space since being informed this space was not included in the previously approved 2nd floor plan. Classes will not be allowed to use the 2nd floor atrium space until further notice. We will seek approval of the space through the proper channels/inspections through DHS, Fire, Environmental, etc...and use of the space will only resume once approved.
