Agency self-reported that a 4 year old child opened the playground gate that leads to the parking lot and exits the playground unsupervised. — Visit: Technical Assistance,Incident or Injury Investigation — Action taken: Agency will be adding an additional latch to the gate that will be keep out of reach of children as well as an alarm that will go off when the gate is opened. They will also asign an educator to be watcher and a player while outside for supervision.
